Pay: Tiered system based on experience and education with incentive options for productivity.

What You’ll Do As a Children’s Behavioral Health Targeted Case Manager:

Work directly with children and families to assess needs, develop service plans, and coordinate supports
-
Provide ongoing case management to ensure children and families have access to behavioral health, medical, educational, and community-based resources
-
Partner with schools, providers, state agencies, and other community organizations to advocate for services and remove barriers

Support families in navigating complex systems and empower them to achieve goals that promote stability and well-being

Monitor progress, update treatment/service plans, and maintain accurate and timely documentation

Deliver services with a trauma-informed, family-centered, and strengths-based approach
Requirements

Bachelor’s degree in Social Services, Psychology, Human Services, or related field (required)

Strong communication, problem-solving, and organizational skills

Ability to work independently and as part of a collaborative team

What You’ll Do

Conduct holistic assessments to identify client strengths, needs, and aspirations

Develop individualized service plans that prioritize mental health and community connection

Support clients in accessing local resources, services, and meaningful opportunities

Advocate for client rights and promote inclusion within community systems

Build trusted relationships with families, providers, and community partners

Monitor progress, adjust plans as needed, and celebrate client milestones

Maintain clear, up-to-date documentation in line with agency standards

Participate in team collaboration, ongoing training, and professional development
What You Bring

Associate’s degree or higher in social work, human services, psychology, or related field

Knowledge of the mental health field

Eligibility for (or current) MHRT-C certification

Job Description: We are seeking a dedicated and experienced Individual Outpatient Mental Health Clinician to join our dynamic team. The successful candidate will provide high-quality, evidence-based mental health services to individuals in an outpatient setting. This role involves conducting assessments, developing treatment plans, and delivering therapeutic interventions to support clients in achieving their mental health goals.
Key Responsibilities:

Conduct comprehensive mental health assessments to determine client needs and develop individualized treatment plans.

Provide individual therapy sessions using evidence-based therapeutic approaches.
