SERVICES

Community INtegration Services

Life can feel overwhelming when you’re managing mental health challenges and trying to get the support you need. Community Integration Services connect you with a case manager who listens to what matters to you and helps you navigate resources, overcome barriers, and work toward your goals.

Depending on your needs, your case manager can help you:

  • Find and connect with housing, food, transportation, and other essential resources.

  • Apply for benefits and understand available supports.

  • Connect with healthcare, mental health treatment, and recovery services.

  • Coordinate care and communicate with providers.

  • Explore education, employment, and opportunities for community connection

  • Advocate for your needs and build confidence navigating complex systems.

You lead the way. Together, we create a plan based on your strengths, priorities, and choices. Case management helps you access and coordinate support; it does not guarantee housing, benefits, or other services.

Outpatient Therapy

Outpatient therapy gives you a space to talk openly, feel heard, and work through challenges with a trained mental health professional. You don’t need to have everything figured out—or be in a crisis—to get started.

Therapy can help you:

  • Navigate anxiety, depression, stress, and difficult emotions.

  • Process grief, loss, trauma, and life changes.

  • Understand patterns in your thoughts, feelings, and behaviors.

  • Build coping skills and strengthen relationships.

  • Explore your identity, values, boundaries, and goals.

Your experiences and choices guide the work. You and your therapist will create a plan based on what matters to you, building on your strengths and moving at a pace that supports your needs.
